Consent Agreement
I acknowledge that I have read and understood this text, and I hereby give my explicit consent to the taking of photographs and/or video recordings of me, as well as any visual content shared by me with the healthcare facility and/or healthcare professional, and to the processing, sharing, and use of visual content that may be produced during such recordings by the aforementioned healthcare facility and/or healthcare professional for promotional and informational purposes within the scope of healthcare services, provided that my identity remains confidential.
Explanations:
- The patient has the right to withdraw this consent at any time.
- This consent relates solely to the processing, sharing, and use of visual data and does not affect the treatment process.
- Every surgical and interventional procedure carries certain risks. The results obtained may vary from person to person, and the same outcomes may not be achievable in every case. Early-stage results may be misleading, and the results shown in visual materials may change over time. Prior to the procedure, the patient is informed by the attending physician.
